This is the first pitfall in the game I think, so it might be a good idea to stick one of those death signs there like you have in later stages.
The first pitfall is near the end of the first level. Either way, those signs in world 2 will be over "Quicker Sand" which is a darker quicksand that you instantly fall through (the graphic for quickersand just hasn't been added yet).

One more block should get cut out of this tree so you can run into it without getting caught up.
I've been meaning to do that but I keep forgetting.

Maybe stick mushroom caps under the floating springs?
You already said that and I already told you that that is a place holder for some bouncy mushrooms that will be added soon.

When entering that pipe it doesn't display the animation, you just disappear.
Yeah, for whatever reason, when you enter a pipe, the player drops to a different layer, so they fall behind some of the tiles. I'll have it fixed soon.

Some tiles here would be cool.
Like the train level, that is a reference to the Wario Land train level, or to be more specific, a glitch in it that trapped you in an invisible room.

The controls lock up after you deal the last hit so it's possible to end up falling into the pit as well and dying.
If I'm not mistaken, the controls lock up after you land from the last hit. Before that you can control yourself so that you land on a solid platform.

Wrong sign?
No, thats the right sign. Right after that is a section that requires you to duck for a while (which would activate Luigi's flashing) and ap rize high above you. In other words, it's a teaching aid.

Is this the only stage with pink blocks left?
Pretty much. I've mainly been using them to block secret exits that lead to the Challenge Levels, that way people can't do them until after they beat a world (so they don't spend a lot of time trying to beat the level, only to find that it's a dead end).

The perspective on these boxes looks weird. They're isometric when nothing else in the stage is. Also even if the rest of the stage was isometric they would still be lined up incorrectly.
Is everything in real life facing the exact same way? Nope. Honestly, I was going to have them at the same angle but it make them look too solid imo. This way prevents that, and it's a little less boring looking.

Sometimes these platforms get out of sync and you can't make the jump.
I've seen this happen, but never to the degree in which you can't make the jump.

You can see the bottom of Mario's feet up top there. I don't think you're supposed to be able to get up there since it makes the level really easy.
Not much I can do since you can't place items off screen in ogmo. It kinda works though since in the end you miss the goldshroom which is the main point (since the goal doesn't lead anywhere).

A spring here would be nice. If you fall you either have to kill yourself and start from the checkpoint or do the entire level over.
Hmmm.... Yeah, I probably will since that section is meant to train you on how to use Super Thin strategically.

I have Waluigi in this screenshot but I'm pointing this out because Wario can't make the jump up to the lock. He can throw the key up and open the door but there's no point to it since he can't reach the exit.
Yeah, TD made that one. He wanted it to be so Wario could get the key easily but couldn't use it, and everyone else had a hard time getting it but they could use it. Once I figured out how to get it, I was fine with it.

Standing on an invisible block.
It's a shortcut warp pipe for testing purposes.

What happened to your fire flower? Either way I can add another torch.
